Welcome to our practice!  We are pleased that you chose our practice for your eye care. At Armesto Eye Associates we strive to provide the highest quality care for you and your family.  In addition to cornea-related specialties we offer everything from routine eye examinations, refractive surgery, glaucoma, cataract and diabetic eye care as well as contact lens services and general ophthalmic surgery. 

We hope the information within our website will be of some help to you and will also assist us in your care.  We respect your time and want to make your visit as efficient as possible. 

MEDICAL INFORMATION

In order for our physician to perform a comprehensive examination, we request that you bring the following items to your first appointment:           

1.      A completed Medical Information form (front and back).

2.      A completed Patient Information form.

3.      All insurance information.

4.      A list of all medications you are taking. 

MEDICAL INSURANCE

Please complete the forms before your visit.  This assists us in maintaining an accurate record of personal and insurance information.  We will file a claim with your insurance based upon the information you provide to us. Please bring all insurance cards with you to your visit to ensure accurate and prompt submission of your claim. 

OFFICE POLICY

We request that you read, sign, and return the Financial Policy. 

CANCELLATION

Our appointments are made on a limited basis.  Please notify us as early as possible if you are unable to keep your appointment.
